IMPORTANT UPDATE:
Mahalo for the outpouring of support as our community rallied together to work through the challenges of the end of the National Dislocated Workers Grant. Through the support of a philanthropic sponsor, we are pleased to announce the re-opening of the Kākoʻo Maui Distribution Center on Monday, March 3rd. All previously-cancelled appointments will be rescheduled. Please allow our team a few days to contact you to reschedule.

Request Forms

Please schedule your appointment by completing the appropriate form. All supplies are on a “first-come, first-served” basis. Completion of a request form does not guarantee that requested items are available.

Short-Term Supplies consist of items such as Clothing, Food, Toiletries and Household Cleaning Items, and are available to those who are indirectly-impacted by the Maui Wildfires.
Long-Term Suppliesare open to those directly impacted by the wildfires in need, specifically households that lost their primary residence in the Maui Wildfires.
Please use this form if you are an organization and/or a DCM requesting supplies for your clients and/or multiple households.
*Our furniture, mattresses and air purifiers have specific donation criteria based on donor intent. We are required to do additional tracking for each item.
